Understanding the Georgia Medicaid Care Pathway
Georgia’s Elderly and Disabled Waiver Program includes CCSP and SOURCE. These programs may help eligible older adults and people with disabilities receive services at home or in the community instead of a nursing facility.

Start the Conversation
Contact Ageless and tell us about current care needs, daily routines, recent health changes, Medicaid status, and the type of assistance needed at home.
Step 2
Review the Program and Referral Pathway
We can explain general program information and help connect your family with Georgia’s Aging and Disability Resource Connection for screening.
Eligibility, level of care, service approval, and provider assignment are determined by the authorized agencies and care coordinators.
Step 3
Complete the Ageless EDWP Inquiry Form
Provide the information needed for Ageless to understand the request and prepare for next steps if services are approved and assigned to our agency.

Services That May Appear on an EDWP Care Plan
- PSS/PSSX: Personal Support Services and Extended Personal Support Services
- ADH: Adult Day Health
- HDM: Home-Delivered Meals
- ALS: Alternative Living Services
- ERS: Emergency Response Services
- SNS: Skilled Nursing Services
- Respite Care
- SFC: Structured Family Caregiving
Ageless may not provide every service listed above. Availability depends on Ageless’s current approvals, the member’s authorized care plan, provider assignment, service area, and staffing.
Understanding CCSP
Community-Based Support for Eligible Georgians
CCSP means Community Care Services Program. It is part of Georgia’s Elderly and Disabled Waiver Program and helps eligible older adults and people with disabilities receive approved services at home or in the community.
CCSP May Include
- Personal support services
- Adult day health
- Home-delivered meals
- Respite care
- Emergency response services
- Alternative living services
- Care coordination and other approved supports
How Ageless Participates
Ageless is an approved CCSP provider. We may accept a member after receiving the referral, approved service authorization, care plan, service location, and other required information.
Services and Medicaid billing cannot begin before authorization and agency acceptance.

Coordinated Support for Eligible Members at Home
SOURCE means Service Options Using Resources in a Community Environment. It is a Georgia Medicaid program that helps eligible older adults and people with disabilities receive coordinated services in their homes or communities.

- Personal support services
- Adult day health
- Home-delivered meals
- Respite care
- Emergency response services
- Alternative living services
- Skilled nursing when authorized
- Enhanced case management and care coordination
How Ageless Participates
Ageless is an approved SOURCE provider. We work with the member’s care coordinator and may begin approved services after the referral, authorization, care plan, service location, and required records have been received and accepted.

SOURCE eligibility and service authorization are determined by Georgia Medicaid and its authorized program partners. Ageless can provide general information but cannot guarantee eligibility or placement.
